Her Power! is July 25th - July 28, 2019 at Camp Fish Tales in Pinconning, Michigan.
To register please mail your completed registration form and payment to Attn: Theresa Metzmaker, MDRC, 3498 E. Lake Lansing Rd. Suite 100, East Lansing, MI 48823. Forms can also be faxed to (517) 333-2677.
Once we have received your payment we will mail or email you the health form packet. You are welcome to send all of the forms in at once.
Registration Cost for Girls with Disabilities
Due to our commitment to make sure that cost is never a barrier to participation Michigan Disability Rights Coalition has established the following suggested registration options for this program. Remember this payment is all inclusive and pays for your food, lodging, personal care attendants, program costs, T-Shirts, and Art Supplies and much higher than the options suggested. Please make checks out to “Her Power” and put “Her Registration” in the memo line.
· Girls that will be paying the fee of the program out of their own money are asked to pay $60
· Families that will be supporting the attendance of a girl are asked to pay $90
· Organizations that will be supporting the attendance of a girl are asked to pay $100
· $100 or more who are able (any payment above $100 is considered a tax-deductible donation)
Michigan Disability Rights Coalition has a fund for girls that need any financial assistance to attend. PLEASE call Theresa Metzmaker 517-333-2477 x 326 if you would like to attend and need help paying to participate.
Support a Girl to Attend!
On the request of parents whose daughters previously attended we are giving families, organizations, and individuals the opportunity to support our scholarship fund. If you would like to pay for another girl to attend, please consider making a donation to Her Power! Checks should be made out the MDRC and please put “Her Power” in the memo line.
When filling out paperwork please keep in mind the following:
HEALTH HISTORY FORM: A health history form must be filled out and signed by the parent/guardian before girls attend camp. A health EXAM is NOT required! If girls have been hospitalized or have been under a Doctor’s care for an illness or injury within the past year that could affect their camp experience, we suggest you have your doctor evaluate the youth and advise you and the camp of any limitations or situations we should be aware of.
OVER THE COUNTER & PRESCRIPTION MEDICATIONS: It is a state law that all medications (including over the counter ones) be kept for the girl by a designated facilitator and camp nurse. This includes things like Tylenol, medicated creams, medicated throat lozenges etc. All medications should be listed on the health history medication sheet, labeled with the girl’s name, and placed in there original containers in a plastic ziplock bag.
Prescription medications must be current, in original container, and prescribed for the girl. If you want the participant to have a different dose or take it at a different time then the label indicates you must sign a note indicating the change. If a girl needs someone to give an injection please notify the facilitator ahead of time. We will be at an outdoor facility which may trigger allergies. It is important to send inhalers, breathing machines etc. EVEN IF SHE HAS NOT HAD A PROBLEM recently. We will call you if your child/minor has a health issue that requires a doctor’s visit, the need to return home, or we need your input to help solve.
ACCOMMODATIONS/ALTERNATIVE FORMATS/DIETARY NEEDS: It is extremely important that facilitators are aware of any accommodations that may be needed by the participant. We ask for accommodation requests ahead of time so we can ensure full participation of each girl and do our best to help meet the needs of every girl attending. Some examples of accommodations include: wheelchair access, ASL interpreter, service dog, ability to access snacks throughout day, large print font, assistance with transfers, vegetarian meals, sugar free foods, etc. Please list any accommodations requests on the accommodation form. If we are unable to provide or have any questions regarding the requested accommodations, we will contact you. Personal Assistants (PA’s) will be provided by MDRC and will be available at all times throughout the event. They are available to assist with daily activities such as transferring, toileting, eating, dressing, etc. All Personal Assistants will be Young Adult Women that have had background checks.
